The actual registration process will take place on the first day your child comes to day camp and will be quick and simple if you've pre-registered your child. If you haven't, the forms below will need to be completed on the first day. After that, each day requires only a simple check-in procedure.

Pre-registration can be done at any time before July 28, and you'll save $10 per child if you pre-register them by July 17. To get started, click on each of the forms below, print them, fill them out, and fax, mail, or bring them in to Brookhaven's office before July 28, along with your non-refundable deposit of $25 for each child. We need one of each form filled out for every child participating in the program. The full balance of the registration fee is due by July 31, 2006.
