Medical Consent Form
This form must be signed by those 18 and older who choose to participate in any Brookhaven Retreat activity. Parents or guardians must sign on behalf of minor children who participate in any Brookhaven Retreat activity. It provides a brief medical history of the participant and gives consent to medically treat individuals, should that become necessary.
Assumption of Risk and Release
Each person must read, sign and return this form to be allowed to participate in any Brookhaven Retreat activities. Please make as many copies of this form as needed and return signed copies to Brookhaven's office upon arrival. Thanks!
